Program description

The FY 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. For each SAC Notice of Funding Opportunity (NOFO), service areas available for competition are announced in the Service Area Announcement Table (SAAT). Throughout the year, we may announce additional service areas in a Service Area Competition-Additional Area (SAC-AA) if an awarded health center cannot continue to provide services in the funded service area.

Competition is limited to announced service areas published in the Service Area Announcement Table. Check the NOFO for available service areas in your region.

What activities and services are funded?

The grant supports comprehensive primary care, preventive services, dental care, and behavioral health services in medically underserved communities.

How competitive is this program?

This is highly competitive with a large funding pool ($470+ million) attracting applications from established health centers and new applicants. Strong evidence of community need is essential.

What is the funding range?

Awards typically range from approximately $1.2 million to $19 million annually, depending on service area size and scope.

💡 Tips for applicants

  • Align your application to announced service areas. Applying for unannounced areas automatically disqualifies you.
  • Emphasize demonstrated capacity to serve medically underserved populations, not just organizational size or infrastructure.
  • Include letters of support from local health departments, schools, and community organizations showing stakeholder buy-in.
  • Document baseline health disparities in your target service area using epidemiological data and community assessments.
  • Build your budget on realistic patient volume projections based on comparable health centers in similar demographic areas.

⚠️ Common mistakes

Not verifying the service area is open for competition in your deadline year. Underestimating the time required to conduct a comprehensive community health needs assessment. Submitting weak or generic letters of support without specific commitments from key partners.
