Funded Projects

Examples of what this program has supported.

FY2025 The Health Center Program provides patient-centered, comprehensive, primary health care to more than 32 million people a year. Nearly 1,400 HRSA-funded health centers operate more than 16,000 service delivery sites in communities across the United States.

Health centers provide services to 1 in 5 people living in rural communities, 1 in 8 children, 1 in 15 adults 65 years and older, and are leaders in the delivery of quality preventive and primary care. In 2024, health centers reported significant quality improvements including a nearly 7% increase in patients with controlled hypertension and diabetes, and over 4.5 million children were screened for weight assessment and nutrition counseling. Through focused quality improvement awards, health centers have also tested and scaled innovative models to improve care in priority areas such as maternal health.
