Program description
The FY 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. For each SAC Notice of Funding Opportunity (NOFO), service areas available for competition are announced in the Service Area Announcement Table (SAAT). Throughout the year, we may announce additional service areas in a Service Area Competition-Additional Area (SAC-AA) if an awarded health center cannot continue to provide services in the funded service area.

💡 Tips for applicants
- Review the Service Area Announcement Table (SAAT) carefully to confirm your target service area is open for competition. Applying to a closed area will result in ineligibility.
- Demonstrate strong evidence of community need, existing organizational capacity, and ability to serve the medically underserved population in your proposed service area.
- Include detailed service delivery plans showing how you will provide comprehensive primary care and preventive services. Be specific about staffing, facilities, and clinical protocols.
- Show financial sustainability and organizational stability. Include letters of support from local health departments, community leaders, and potential referral partners.
- Align your application with HRSA's priorities around health equity, workforce diversity, and innovation in underserved care delivery.
⚠️ Common mistakes
Applying to a service area not listed in the current SAAT competition announcement. Not demonstrating adequate organizational capacity to deliver comprehensive services. Underestimating the need and feasibility narratives for your service area and population.
