Program Objective
The Centers for Medicare & Medicaid Services (CMS), through its Center for Medicare and Medicaid Innovation (Innovation Center), seeks applications to the Maternal Opioid Misuse (MOM) model. The MOM model is the next step in CMS’ multi-pronged strategy to combat the nation’s opioid crisis. The model aims to address fragmentation in the care of pregnant and postpartum Medicaid beneficiaries with opioid use disorder through state-driven transformation of the delivery system surrounding this vulnerable population. By supporting the coordination of clinical care and the integration of other services critical for health, wellbeing, and recovery, the MOM model has the potential to improve quality of care and reduce costs for mothers and infants

How to Apply
Award Procedure
1. Successful applicants will receive a Notice of Award (NoA) signed and dated by the CMS Grants Management Officer that will set forth the amount of the award and other pertinent information. The award will also include standard Terms and Conditions. Applicant should be aware that special requirements could apply to cooperative agreement awards based on the particular circumstances of the effort to be supported and/or deficiencies identified by the review panel.
2. Award funds will be distributed through the Payment Management System.

Description
The MOM model leverages Innovation Center authorities and state flexibility to address fragmentation in the care of pregnant and postpartum Medicaid beneficiaries with opioid use disorder. To foster integrated and coordinated care for the model population, CMS supports awardees to design and implement state-specific interventions through funding for 1) infrastructure and capacity development, 2) care delivery, and 3) milestones tied to key quality metrics. During the model’s five-year performance period, responsibility for funding the care-delivery innovation has transitioned to states, with the ultimate goal of sustaining successful payment and care-delivery strategies through incorporation into their Medicaid programs. CMS currently funds eight renewable one-year cooperative agreements to State Medicaid agencies. No additional awards are anticipated.

Allowed Uses
The MOM model provides three distinct funding opportunities for awardees:
• Implementation Funding will be available annually during each year of the model to support infrastructure and capacity-building.
• Transition Funding will be available only during the Transition Period and only to support intake, assessment, and creation of a treatment plan (payment available once per pregnancy per beneficiary) and coordination, engagement, and referral activities (payment available on a monthly basis for up to 12 months for each beneficiary who receives these services) not otherwise covered by the State Plan. Model funds may not be used to supplant or duplicate existing funding sources. The state must take over these costs in alignment with its Coverage and Payment Strategy by Year 3 of the model. Transition Funding is restricted and not guaranteed.
• Milestone Funding will be available during the Full Implementation Period based on an awardee’s demonstrated performance on pre-determined quality milestones. Milestone Funding is restricted and not guaranteed.
Applicants must describe in detail how they will use Implementation Funding and Transition Funding to implement the model. In addition to the detailed justifications for use of Implementation Funding and Transition Funding, each applicant should provide a plan for how Milestone Funding, if unrestricted, would be used to support the goals of the MOM model.

All applicants under this announcement must have:
• An Employer Identification Number (EIN), otherwise known as a Taxpayer Identification Number (TIN), to apply.
• All applicants must have a Dun and Bradstreet (D&B) Data Universal Numbering System (DUNS) number.
• The applicant must also register in the System for Award Management (SAM) database in order to be able to submit the application.
