FY2025 VA provided 296,520 headstones, markers, and medallions and 495,210 Presidential Memorial Certificates.

Program Objective

Provide Government-furnished headstones and markers to mark the graves of and memorialize Veterans and other eligible individuals in national, state/tribal, or private cemeteries around the world. Provide Government-furnished medallions for placement on privately-purchased grave markers of Veterans. Provide Presidential Memorial Certificates to families and friends to honor the Veterans’ military service.

A decedent's family member, personal representative, or other authorized applicants may submit claim forms for headstones, markers, medallions, and Presidential Memorial Certificates, as defined by Title 38 authorities.

Description

The purpose of Memorial Products Service (MPS) is to provide Government-furnished headstones, markers, and medallions for eligible Veterans and dependents around the world and Presidential Memorial Certificates to families and friends to honor the Veterans’ military service.

Allowed Uses

Government-furnished headstones and markers and medallions are provided for Veterans who are discharged under conditions other than dishonorable and other eligible individuals and are used to mark gravesites and memorialize eligible decedents in national, state/tribal, and private cemeteries. Presidential Memorial Certificates are issued for Veterans who are discharged under conditions other than dishonorable.

Required Documentation

A copy of the deceased Veteran's discharge certificate (DD Form 214 or equivalent) or a copy of other official document(s) establishing qualifying military service. Completed forms: VA Form 40-1330, Claim for Standard Government Headstone or Marker; VA Form 40-1330M, Claim for Medallion for Placement in a Private Cemetery; VA Form 40-0247, Presidential Memorial Certificate Request Form. This program is excluded from coverage under 2 CFR 200, Subpart E - Cost Principles.
