FY2025 Implemented Integrated Maternal Health Services models and enrolled clients within the target communities.

Recruited providers and practices within identified regions to participate in the models, including obstetricians and certified nurse-midwives to provide clinical care.

Collaborated with Federally Qualified Health Centers to enroll clients.

Provided behavioral health services to pregnant women, which included depression screening and referrals, and integrated these services into the pregnancy medical homes.

Collaborated with the State Medicaid agency to enroll pregnant women with the greatest needs

Program Objective

The Integrated Maternal Health Services program fosters the development and demonstration of integrated maternal health services models, such as the maternity medical home, which is modeled after the patient-centered medical home. The purpose of the program is to improve maternal health outcomes in the United States by increasing access to quality, equitable, comprehensive care for pregnant and postpartum women who experience health disparities and have limited access to basic social and health care services.

Eligible entities for the Integrated Maternal Health Services program include any domestic public or private entity, including Indian tribes or tribal organizations, as well as domestic faith-based and community-based organizations.

How to Apply

Award Procedure

All qualified applications will be forwarded to an objective review committee. Based on the recommendations of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions. Notification is made in writing by a Notice of Award.

Decision Timeline

  • Approval: From 120 to 180 days

Final decisions are made 4 to 6 months after receipt of applications.
