Program description
This Ohio Ultrasound Grant Application (“Application”) is due by August 14, 2026. Applicant (“Grantee”) must use this form to apply for State Fiscal Year (SFY) 2027 (July 1, 2026 to June 30, 2027). Applicant-Grantee must completely fill in the requested information and include all other required documentation within the Application to the Ohio Department of Children and Youth (DCY). The Application will only be considered when all required documents and information has been provided by the deadline. A fully executed Application will serve as the Grant Agreement between DCY and the Grantee. Ohio Amended House Bill 479, changed the design of the Ultrasound program. Instead of DCY purchasing ultrasound machines, DCY will now provide direct grants to grantees to purchase their own diagnostic ultrasound machines with technology of their choice.
