Program Objective
These cooperative agreements are intended to increase the number of evidence-based falls prevention programs available to older adults, while concurrently increasing the sustainability of these proven programs in the aging and disability networks. This funding also supports a cooperative agreement for a National Falls Prevention Resource Center.

Eligible applicants for grants are domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education. This funding opportunity is designed to develop capacity for, bring to scale, and sustain evidence-based falls prevention programs to help older adults remain independent and live in the setting of their choice.

Allowed Uses
Grant funding could potentially be used for agreed-upon activities associated with the following goals: Goal 1: Significantly increase the number of older adults and older adults with disabilities at risk for falls who participate in evidence-based community programs to reduce falls and falls risks.
Goal 2: Implement innovative funding arrangements, including contracts and collaborations with one or more sustainability partners, to support evidence-based falls prevention program(s) both during and beyond the grant period. Additionally, embed the program(s) into an integrated, sustainable evidence-based prevention program network via centralized, coordinated processes.
Required Documentation
The successful applicant will be an organization that meets the following criteria:
1. Proposed only evidence-based falls prevention programs that are on the list of pre-approved programs in the Funding Opportunity Announcement
2. Provided signed letters of commitment from key partners and collaborators named in the proposal.
3. Included a Budget Narrative/Justification.
