Program Objective
The purpose of this program is to fund National Networks to fund, manage, support, and monitor sub-recipients to address health disparities and implement evidence- and practice-based strategies that reduce health disparities for intervention population(s) experiencing high burden of disease or risk factors. The intervention population is the population within the selected geographic area that applicants will reach with proposed program activities. They are to contribute to the development and implementation of a community health action plan to address chronic disease health disparities.

How to Apply
Award Procedure
After review and approval, a Notice of Award (NoA) will be prepared and processed, along with appropriate notification to the public. Initial awards provide funds for the first budget period (usually 12 months) and the NoA will indicate support recommended for the remainder of the project period, allocation of Federal funds by budget categories, award requirements, and special conditions, if any.

- Approval: From 90 to 120 days
- Renewal interval: From 120 to 180 days

Description
The purpose of this program is to support national organizations that work with and oversee local partners to improve health outcomes in communities with a high burden of chronic disease. These organizations will help communities plan, coordinate, and carry out proven strategies that address key risk factors, strengthen community capacity, and improve overall health. The program emphasizes community-driven planning and action to support lasting improvements in chronic disease prevention and management.

Allowed Uses
Project funds may be used for costs associated with planning, organizing, conducting, implementing, managing, and coordinating all recipient requirements of the Notice of Funding Opportunity announcement. Applicants must adhere to all CDC guidelines regarding allowable and unallowable expenses and activities. CDC will have substantial involvement with the award recipient during performance of the funded activity.
