This grant is for research institutions and medical centers proposing comprehensive regional research centers focused on multiple chronic diseases (MCDs).

Applicants must be able to form collaborative teams with expertise in clinical research, health services, behavioral interventions, implementation science, and community engagement. Institutions should have capacity to conduct research that addresses two or more co-occurring chronic conditions (such as cardiovascular disease, cancer, diabetes, or respiratory disease).

The grant prioritizes research advancing prevention, diagnosis, treatment, and management of MCDs among populations experiencing health disparities, including pediatric and adult populations. Funded projects will develop novel research models, test interventions, or implement evidence-based strategies in community or healthcare settings.

Program description

This forecast is to renew the Notice of Funding Opportunity (NOFO) for the Multiple Chronic Disease Research Centers (RFA-MD-21-007). This initiative will support existing and new regional comprehensive research centers to optimize prevention, diagnosis, treatment and management of multiple chronic diseases (MCDs) and to improve health outcomes among pediatric and adult populations experiencing health disparities. The centers will focus on innovative research that: (1) develops, tests, and evaluates novel models or strategies, and/or (2) implements effective interventions in various settings such as community or healthcare settings/systems.

Chronic diseases, such as cardiovascular diseases, cancer, diabetes, and chronic lower respiratory diseases, are among the leading causes of morbidity and mortality in the U.S. The prevalence of chronic health conditions among U.S. children and adolescents (especially cardiometabolic diseases) is also increasing. The effective integration and implementation of evidence-based interventions for both adults and children with comorbid chronic conditions remains understudied and is a priority. Each Center must address two or more chronic conditions that commonly co-occur and/or share common social context, etiological pathways, or risk factors and management strategies.

Research projects that advance the Make America Healthy Again Commission goals, with a focus on addressing disparities in the prevention or effective diagnosis, treatment and management of chronic conditions will be prioritized. In addition, research design and methodologies that are replicable, reproducible and generalizable are priorities.

Applications are not being solicited at this time. Notice is being provided to allow potential applicants sufficient time to develop meaningful collaborations and responsive projects. This NOFO will utilize the P50 (Critical Trial Required) activity code. Investigators with expertise and insights into the proposed scientific areas are encouraged to begin to consider applying for this new NOFO. In addition, collaborative investigations combining expertise in clinical, health services, behavioral interventions, implementation science, community engaged research, and clinical care will be encouraged, and these investigators should also consider applying for this NOFO.

FAQ

What types of institutions can apply for this grant?

Research institutions, medical centers, universities, and regional health systems can establish comprehensive research centers. Applicants must demonstrate capacity for collaborative, multidisciplinary research teams.

What chronic diseases should my center focus on?

Centers must address two or more chronic conditions that commonly co-occur. Examples include cardiovascular disease, cancer, diabetes, and chronic respiratory diseases. Choose conditions sharing risk factors, pathways, or management strategies.

Is cost-sharing required?

No cost-sharing is required for this grant. However, demonstrating institutional commitment and leveraging existing resources strengthens competitiveness.

When is the application deadline?

The deadline is July 25, 2025. This is a fixed deadline. Applications are not being solicited at this time; the forecast period allows time to develop collaborations.

What research approaches are prioritized?

Research must be replicable, reproducible, and generalizable. Implementation science, community-engaged research, and health equity approaches are strongly encouraged.

💡 Tips for applicants

  • Start building collaborative teams now. The forecast period is designed to allow time for meaningful partnerships across clinical, behavioral, and implementation science expertise.
  • Focus on two or more chronic diseases with clear connections—shared risk factors, common populations, or overlapping management strategies strengthen your proposal.
  • Emphasize health disparities and equity. Research advancing prevention and treatment among underserved populations (pediatric and adult) aligns with program priorities.
  • Design research that is replicable and generalizable to other settings. Avoid overly site-specific models; demonstrate broader applicability.
  • Use community-engaged and implementation science methods. These approaches directly address how to move evidence-based interventions into real-world healthcare and community settings.

⚠️ Common mistakes

Proposing a single chronic disease rather than addressing comorbidities. Centers must clearly articulate why two or more conditions should be studied together.

Underestimating the importance of health disparities and equity. If your proposal does not explicitly center prevention, diagnosis, and treatment of MCDs in underserved populations, competitiveness suffers.

Lacking multidisciplinary collaboration. Applications without demonstrated partnerships across clinical, behavioral, implementation science, and community engagement are weaker.
