💡 Tips for applicants
- Emphasize partnerships early in your application. CDC cooperative agreements require active collaboration with public health and healthcare partners.
- Focus on chronic diseases not covered by existing disease-specific CDC programs. Identify your target population clearly.
- Include concrete plans for surveillance data dissemination. Show how you'll reach public health professionals and the general public.
- Demonstrate evidence-based approach throughout. Document which practices and education strategies have research support.
- Detail your capacity for national scope work. NCCDPHP prioritizes organizations that can implement and disseminate at scale.
⚠️ Common mistakes
Proposing work on disease-specific conditions already supported through dedicated CDC programs reduces competitiveness. Weak partnership letters or unclear collaboration roles decrease funding prospects. Vague surveillance plans without specific data sources or dissemination timelines signal weak implementation capacity.
