Program Objective
In 2001, Congress provided funding to CDC to establish the Paul Coverdell National Acute Stroke Registry, named after the late US Senator Paul Coverdell of Georgia, who suffered a fatal stroke in 2000 while serving in Congress. To improve the quality of acute stroke care and health outcomes for acute stroke patients. Program activities will address the continuum of care from onset of stroke through rehabilitation and recovery, focusing on health systems change and community and clinical linkages.

Eligibility is limited to state health departments (to include the District of Columbia) with heart disease and stroke prevention programs for this cooperative agreement.
State health departments are the only agencies who are uniquely positioned to develop strong state level task forces to develop these stroke systems of care that can be used to focus on an comprehensive approach to improving quality of care at all points along the continuum of care that will have the largest reach and impact on decreasing morbidity and mortality from stroke, reducing disparities in the delivery of care, and improving outcomes

Description
This iteration of Coverdell supports state stroke systems in partnerships with learning collaboratives or coalitions to improve state-level stroke care for those at the highest risk. Recipients will be required to execute a dual approach to addressing stroke in the coordinated systems of care and the implementation of prevention activities in community settings.

Funds may be used to: Develop and maintain partnerships; implement and support the operation of a hospital based stroke registry; and Develop and maintain a quality improvement program to increase the quality of care in all applicable components.
