Program Objective

The purpose of this five-year program is to reduce the rates of disability and death associated with chronic disease in the U.S. Affiliated Pacific Islands, U.S. Virgin Islands, and Puerto Rico. The U.S. Affiliated Pacific Islands include American Samoa, Commonwealth of the Northern Mariana Islands, Guam, Federated States of Micronesia, Republic of the Marshall Islands, and Republic of Palau. Evidence-based strategies to reduce tobacco use and secondhand smoke exposure, prevent and improve management of diabetes and cardiovascular disease (CVD), improve availability and quality of data to inform programs, including maternal and child health and oral health, and monitor progress will be supported.
Cross-cutting and unified approaches to address chronic disease and their risk factors, with an emphasis on tobacco use, diabetes, CVD, maternal and child health, and oral health will be supported. The purpose of this program is:
a) To reduce tobacco use and secondhand smoke exposure
b) To improve prevention and management of diabetes
c) To prevent persons at risk for pre-diabetes from developing diabetes
d) To improve prevention and management of high blood pressure and high cholesterol
e) To collect data to inform programs and policies that aim to improve maternal and child health
f) To better address oral health needs

Description

This program will support public health programs to prevent and control chronic disease and associated risk factors in American Samoa, Commonwealth of the Northern Mariana Islands, Guam, Federated States of Micronesia, Republic of the Marshall Islands, Republic of Palau, U.S. Virgin Islands, and Puerto Rico. In recognition of their unique challenges and needs including widely dispersed small populations, small workforce, geographically remote locations, over-burdened health systems addressing both non-communicable diseases and infectious disease outbreaks, and natural disasters, this program will support the implementation of strategies in the four domains of chronic disease prevention that are adapted to be culturally relevant and appropriate. It supports evidence-based strategies to reduce tobacco use and secondhand smoke exposure, prevent and improve management of diabetes and CVD, improve availability and quality of data to inform chronic disease programs, including maternal and child and oral health, and monitor health trends and progress.

Use of Funds

Allowed Uses

Funds can be used to pay overhead, travel costs, and other expenses directly related to achievement of the strategies and activities identified to fulfill the obligations specified in the notice of funding. Funds can be used for salaries for personnel to plan, implement, monitor, and evaluate program activities; for contracts or agreements to implement and evaluate program activities, including assessments and convening coalitions; contracts or agreements with community-based organizations or coalitions to provide health education and lifestyle support programs, or to improve the health of the physical environment (for example to improve access to healthy food, physical activity, or decrease tobacco exposure); or other activities that promote or support healthy behaviors and improved health in the population. Funds can be used to pay for activities and supplies necessary to provide the services outlined here, including self-blood pressure monitoring equipment. Funds can be used to develop and maintain data collection systems to collect and report biomeasures and behavior information and for monitoring systems for chronic disease.
